Please indicate whether your child will need assistance from staff when applying spray-on sunscreen during program hours or outdoor trip.Does your camper need assistance with applying “SPRAY-ON” sunscreen during program hours or outdoor trip activities?.This certificate must be completed and returned to staff prior to your child’s participation in the day camp program. If you answer No, or Yes, the student is home-schooled or was in a school other than the state of Maryland, then the Maryland DHM Immunization Certificate will print out with your Day Camp Confirmation receipt.Yes – student is home-schooled or was in a school other than in the state of Maryland school.If you answer Yes – and the student was in a Maryland public or private school, you will not need to complete a Maryland DHM Immunization Certificate.Yes – student in a Maryland public or private school.If you answer No, you will not need to complete any additional health forms. This form must be completed and returned to staff prior to your child’s participation in the day camp program. The Health-Medication Form will be printed with your Day Camp Confirmation Receipt at the end of the registration.Participants who have health issues such as allergies or other illness, take medication (prescription or over-the-counter), or use emergency medical devices such as inhalers or Epi-pens should answer Yes. ![]() (See page 4 of the camp brochure or online flip book.) Simply register your child for the section titled “Summer Program Participant Information” (16600-300A) and answer the required questions. Beginning February 15th, there will be an Activity Section available on the PARKS DIRECT registration system to collect this information. We require all Summer Program participants to provide us with answers to a few basic questions to protect their health and safety. If you have questions or need help before, during, or after registration please call the Customer Service Help Desk at 30 or email us at Summer Programs Participant Information Children may not attend any session without completed information and forms on site, no exceptions. It is the parent/guardian’s responsibility to provide the answers to the below questions and deliver all completed forms to each program location by the June 1 deadline. To comply with the state of Maryland’s Youth Camp Standards, the Department of Parks and Recreation requires certain information and forms from its participants. Campers may not attend any camp sessions without completed forms on site.Ĭompleted forms are due by June 1, no exceptions. It is your responsibility to deliver all necessary forms to each campsite. Completed forms must be turned in prior to the first day of the camp session.
